Liverpool Ladies midfielder Caroline made her senior debut for the national team at just 17 in June 2013, when she played in a 3-2 friendly win in Iceland.
She celebrated reaching the 50-cap mark with a victory over the Poles as Scotland bounced back from their defeat to group leaders Switzerland on Thursday.
The hosts had goalkeeper Lee Alexander to thank for some fine saves, including from Ewa Pajor's 55th minute penalty, that kept them in the match before Paulina Dudek was sent off for a second booking with 15 minutes left.
Weir, who has represented Scotland at all youth levels, and her team-mates took advantage and made the breakthrough thanks to Zoe Ness' header.
Further goals from Claire Emslie and Erin Cuthbert sealed the victory to keep Scotland six points behind the Swiss, having played a game fewer.
Their next matches will take place in June, when they play at home to Belarus and away to Poland in a World Cup qualifying double-header.
